Donate a Pacemaker

If you are interested in donating a pacemaker through My Heart Your Heart, please follow the steps below:

1. Download the Pacemaker or Implantable Cardioverter Defibrillator (ICD) Donation Consent form
   • Living Donor consent form
   • Deceased Donor consent form



2. Request a postage-paid shipping box to send the pacemaker to University of Michigan. On the form, please select one option: Funeral Home, Health Care Facility or Private Citizen.
   Please allow 2-3 weeks for delivery of the shipping box or envelopes.
