Prior to your first visit, you will need to print off the intake forms. These forms are designed to communicate much of your medical history including current medications, supplements, herbs, conventional and alternative forms of therapy (the intake forms are extensive and need to be filled out at your home prior to your first visit).
Please bring any current medications and/or supplements you may be taking with you to your first visit.
Your first appointment will be 90 minutes for an Integrative Medicine Consultation and 60 minutes for an Integrative Family Medicine visit. Once you arrive at the clinic and have checked in, you will be met by Integrative Medicine Medical Assistant who will do an initial intake and measure vital signs. The physician will review your medical information, listen to your story, and perform a directed physical exam.
On your second visit, which is usually 60 minutes, a unique individualized optimal health plan will be presented to you addressing the mind, body, spirit and emotion.
Your optimal health plan integrates conventional and alternative therapies that best serve the individual's needs. Goals include simplification of regimens, empowering individual patients and communication between providers of care.
- Recommendations are to be gradually incorporated.
- Discussion of the plan with other treating physicians and practitioners is encouraged.
- Recommendations may include referral to practitioners in the University of Michigan, Ann Arbor, or other communities.
When instituting lifestyle changes, regular follow up visits support success. Discuss the best frequency of follow up visits for you with your physician. Follow up visits are generally 30 or 60 minutes in length.
