Program Participation

Entry into the UMHS-CGSP is a three-step process. Each step is designed to include the individual as an active participant in their treatment and care. (Completion of all three steps is necessary for participation in the Program.)

Step 1 - Intake: The comprehensive telephone intake (usually 30 minutes long) is designed to gather medical and psychosocial history, and request previous medical and mental health care records.

Step 2 - Evaluation and Consultation: This process consists of 3-6 sessions, with the first session often a double session, conducted by a member of the UMHS-CGSP Mental Health Team. Cases are presented for review, and individual recommendations are made regarding program participation.

Step 3 - Coordinated Care Plan: This Plan is a set of goals, jointly set and mutually agreed upon between the Program Participant and their UMHS-CGSP mental health professional.

Subsequent referrals to the following primary services are available:

  • Primary Medical Care, including Hormonal Therapy
  • Speech/Language Therapy
  • Dentistry
  • Hair Augmentation/Transplants
  • Cosmetic and/or Reconstructive Surgery
