Patient Packet and Consent Form
Protecting your privacy is very important to us. We will take a number of steps to protect your confidential information. These steps include:
- Assigning you a unique identification number that will be used on all study documents and samples. This number is not the same as your UMHS patient registration number.
- Storing all research documents in locked file cabinets in the research study office. No one but research study staff will have access to this information.
Your medical chart will indicate only that you consented to participate in our study. It will not include specific results of our examinations.
