Survival Flight is committed to providing excellent patient care and excellence in customer service. In an effort to improve our service we have developed a quick survey for our patients and their family members. We are asking that you take the time to fill out this brief survey and tell us what you think. We want to improve and you can help us by letting us know what we are doing right and where we can improve.
This survey is completely anonymous and no personal information is required. All we ask is for the date of the transport and the referring hospital or emergency service that you were referred by.
We sincerely wish to improve our service and depend on feedback from our patients and their families to help us achieve our goal of excellent patient care and customer service.
