Clinical Scenarios: Low Urine Output

  • Questions:
  •     - How far post-op?
  •     - What is the baseline renal function?
  •     - On any nephrotoxic drugs?
  •     - Does patient have a foley? (if not, need one?)
  •     - Is the foley working?
  •  
  • Workup:
  •     - Vital Signs (pulse, BP, I/O)
  •     - BMP
  •     - CBC
  •     - Urine studies (maybe)
  •  
  • Goals of Management:
  • Is the patient bleeding?
  •     - Do they need to go to OR?
  • What is the patient’s volume status?
  • Is there an intrinsic renal problem?
  • Increase urine output:
  •     - NS 500-1000mL bolus
  •     - Increase maintenance fluids
  •     - Reassess frequently
  • Lasix is not an intern maneuver
