Physical Exam: Trauma Exam/Assessment

  • A: Airway- patient speaking? airway patent? airway secure?
  • B: Breathing- breathing spontaneously? rate and labor of breathing?
  • C: Circulation- pulses palpable? blood pressure? RRR?
  • D: Disability- GCS
    • - Motor: no response (1), decerebrate (2), decorticate (3), withdraws to pain (4), localizes pain (5), obeys commands (6)
    • - Verbal: no response (1), basic sounds (2), inappropriate words (3), confused (4), oriented (5)
    • - Eyes: no response (1), open to pain (2), open to speech (3), open spontaneously (4)
    • -Minor ≥13, Moderate 9-12, Severe ≤8
  • E: Exposure- remove clothing, assess for injuries, roll patient (stable C-spine), cover with blankets (hypothermia)
  • TRIAD- hypothermia, acidosis, coagulopathy
