Referrals
How to refer
Referrals are welcomed from physicians and insurers. For medically-based communication or swallowing evaluation, a physician's referral is required. Individuals requesting accent reduction evaluation/treatment may self-refer.
Internal Referrals
Referral from within UMHS may be tubed to: D-7 for adult patients and to J-3 for pediatric patients or faxed to 615-1532.
External Referrals
Referrals from outside of the Health System may be faxed to (734) 615-1532 or mailed to:

Location/Times
The Adult In-Patient and Out-Patient services at University Hospital are located on the first floor of University Hospital, 1D203. The clinic hours are 8 a.m. to 5 p.m., Monday-Friday. link to map
Pediatric In-Patient and Out-Patient services are located on Floor 2 of C.S. Mott Children’s Hospital, F2593 in the Department of Pediatric Physical Medicine and Rehabilitation. Clinic hours are 8 a.m. to 5 p.m., Monday- Friday. link to map
Outpatient services at MedRehab, and the Pediatric and Adult NeuroRehabilitation Programs at MedRehab are located in the Briarwood Mall area, 355 Briarwood Circle. Outpatient program hours are 8 a.m. to 5 p.m., Monday through Friday. link to map
The Vocal Health Center is located at 19900 Haggerty Road. The Vocal Health Team is typically available on Wednesdays. link to map
