Title: Capitalize the entire title.
Authors: Initials or first names should precede the last name. Omit degrees and titles. Underline the name of the author who will present the abstract. Indicate the name and location of the institution where the work was done.
Printing: Print abstract single-spaced (black ink) on this form. Use a carbon ribbon typewriter, laser or ink-jet computer printer only. Do not use fonts smaller than 12 point; they may be illegible when published. Hand print, in black ink, those symbols not available on your printer or typewriter.
Margins: Leave no top or left margins. Short abstracts should not be centered within the abstract space.
Mailing: Do not fold the abstract form. Mail the original plus five photocopies of both sides of this form using cardboard backing to address listed below:
