Denise G. Tate,
PhD, ABBP, FACRM
Project Co-Director: University of
Michigan Model SCI Care System
Professor, Clinical Psychologist and Associate
Chair for Research, Department of Physical
Medicine and Rehabilitation, University of Michigan.
Dr. Tate has been with the University of Michigan Model SCI
Care System since 1986. Her primary research interests lie
in the areas of health outcomes, quality of life, and
adaptation to injury. She has over 70 peer reviewed
publications. Dr. Tate has overseen the post-doctoral
fellowships of over 50 post-doctoral and physician research
fellows. During the course of her career,
Dr. Tate
has received eight awards for the excellence of her
research, including most recently the ACRM
Stanley J. Coulter Lecturer from the
American Congress of Rehabilitation. In
addition to serving as the Project Co-Director for the U-M
Model SCI Care System, Dr. Tate is the Principal
Investigator for two grants funded by the National Institute
on Disability and Rehabilitation Research and one funded by
the National Center for Medical Rehabilitation Research.
Dr. Tate also provides care to all of the people with SCI
after they are admitted to the Adult Rehabilitation Unit at
the University of Michigan Health System along with her
clinical fellows. |

Anthony Chiodo, MD
Project Co-Director: University of Michigan Model SCI Care
System
Clinical Associate Professor, Department of Physical
Medicine and Rehabilitation, University of Michigan
Dr.
Chiodo recently became a Co Director for the University of
Michigan Model SCI Care System. He has been the Clinical
Director of Spinal Cord Injury Medicine at the University of
Michigan since 2002. He has worked on the U-M Model SCI Care
System research projects Wellness in SCI and Predictors of
Health, and Quality of Life in Persons with Ventilator
Assistance after SCI. He has published several manuscripts
on pain management, wellness in SCI, and is preparing
several manuscripts ventilator weaning after SCI. He has
served as a Co-Investigator for two clinical trials of
Fampridine’s efficacy after SCI: as treatments for
spasticity and for restrictive lung disease. In addition to
serving as Co-Director for
U-M Model SCI Care System,
Dr. Chiodo is continuing his research on and pulmonary
function after SCI.
Dr.
Chiodo is the primary attending physician for SCI services in
the Adult
Rehabilitation Unit at the University of Michigan Health System,
coordinating all clinical services.

Martin Forchheimer, MPP
Senior Research Associate and Co-Investigator
Mr. Forchheimer has been with the University of Michigan
Model SCI Care System since 1989. He is the Director of
Research for the project. Mr. Forchheimer serves as the
Principal Investigator for two of its five site-specific
research projects and is the Coordinator for its
collaborative study led by Craig Hospital. Mr. Forchheimer’s
primary research interests lie in the areas of outcomes
research, community integration, and measurement. He has
written over 35 peer-reviewed publications. |

Claire Z. Kalpakjian, PhD
Project Manager and Co-Investigator
In addition to her administrative duties, Dr. Kalpakjian
plays a key role with the project’s dissemination and
research. Prior to joining the project, Dr. Kalpakjian was a
post-doctoral fellow with the Department of PM&R. Her
primary research interests lie in the area of women’s health
and spirituality.
She is
the Principal Investigator for a three-year study on
Menopause and SCI for which Dr. Rodriguez is the
Co-Investigator. She is in the second year of a
National
Institutes of Health Career Development Award in which she
is continuing her work on women’s health and SCI. |

Jane M. Walters, MLS
Research Associate and Data Collector
Ms. Walters started with the University of Michigan Model
SCI Care System in 1990 as a Research Assistant. As the Data
Collection Coordinator, she is responsible for ensuring that
over 15 years of participants are interviewed at appropriate
times and that their data is kept confidential. She oversees
database activities for the project and is in charge of
interactions with the National Spinal Cord Injury
Statistical Center. Ms. Walters’ background as a librarian
and computer programmer has played a role in keeping the
collected information and the databases organized and
correct. She has recently become a secondary author for the
project’s research papers and presentations. |

Kay Morefield, AGS
Research Secretary
Ms. Morefield has served as the Research Secretary for the
U-M Model SCI Care System since 2002. She schedules meetings
and takes minutes, assists with coordination of conferences
and presentations, processes purchasing requisitions, and
assists with preparation of the project’s manuscripts and presentations.
In addition to her work on the project, Ms. Morefield serves
similar duties for Dr. Tate’s Advanced Rehabilitation
Research Training Program. |
