Divisions: Allied Health Education

The Occupational Therapy and Physical Therapy Division at University of Michigan Health System sponsors continuing education courses for therapists, assistants, and other health care professionals. Every course is designed to meet individual educational needs in clinical practice and health care delivery.

Registration

If you are interested in attending one of our courses, please print and fill out this registration form (pdf) . Registration fee must accompany the registration form. Hotel accommodations and maps of the area will be sent with your confirmation letter. You will not be registered for the course until you receive confirmation that verifies your registration. If you do not receive them, please call Darci Watson at 734-936-4376.

If you wish to pay by credit card, please fill out the credit card authorization form (pdf) and send it in with your registration form.

Accommodations

Discounted rates are available at nearby hotels, upon availability. Hotel information is included in all confirmation letters, and participants may call Darci Watson for additional information.

Cancellation Fee

Registration fee, less a $75.00 administration fee, is refundable if we receive a written cancellation notice three weeks prior to the course. No refunds will be given after this date.

We reserve the right to limit the number or participants or cancel the course if deemed necessary due to lack of registration. In the event that a minimum number of participants are not registered within 30 days of the course date, the course will be canceled and all registrants will receive a full refund.

We advice that you DO NOT make any airline reservations until you have received confirmation that you are registered for the course. UMHS OT/PT Division will not be responsible for the refund of transportation or hotel expenses.
