Welcome to the University of Michigan CMS Physician Group Practice Demonstration website.

On April 1st, 2005 the University of Michigan Faculty Group Practice joins ten other institutions across the nation in participating in the CMS Physician Group Practice Demonstration Project.

This program, conducted by the Centers for Medicare and Medicaid Services (CMS) is designed to improve the coordination and quality of care by:

•  Increasing patient education

•  Improving self-management of disease and/or chronic conditions

•  Assisting with care coordination

•  Enhancing patient support

•  Improving communication with care providers

This U-M program, a collaboration of its hospitals, health centers and faculty Group Practice, will phase in quality measures that focus on common chronic illnesses and preventive care measures during the three-year demonstration.
