Registry Data Submission Methods
Data submission on pediatric pacemaker patients is welcomed from all pediatric cardiology centers. Patient confidentiality is maintained and only the submitting center has access to the individual patient records. Submitting centers can request pooled information concerning specific questions on request by sending the questions to be addressed to the Registry Director.
Data can be submitted using the registry forms either by mail or electronically. The registry forms are available in a text file that can be printed or used as an E-Mail form . Select the option "Download to disk" in your browser and then click on plain text file. This file contains a complete set of forms and can be opened in a word processor to print the foms. Descriptions of the forms follow below. The completed forms can be mailed to the Registry Director at the address on the Home Page or by electronic mail to GSerwer@umich.edu.

If you have a forms capable Web browser, forms can be filled out on line and submitted automatically by selecting the appropriate page below.
There are four separate forms to be used depending on the type of information to be submitted.
- Initial Patient Entry
- This form is used to register a patient new to the Registry or in the event of the death of a previously registered patient. You do not need to use this form when registering new devices implanted in previously registered patients.
- Generator Implant Entry
- This form is used to register a new generator including the initial programming of the generator.
- Generator Explant Form
- This form is used to indicate that a previously registered generator has been removed from service. The reason for the generator being removed and the last programmed settings are to be included.
- Electrode Entry Form
- This form is used to register both new electrodes and to indicate that a previously registered electrode has been removed from service. The threshold data obtained at implant or at the time the electrode was retired is to be included.
