Referring Physicians

Dear Colleague:

Thank you for referring your patient to the University of Michigan Health System’s Department of Orthopaedic Surgery. We value our relationship with you and appreciate your confidence in our service and staff.

It is our goal to provide your patient with the highest quality of care in the most efficient manner. To expedite the referral process, we would appreciate your assistance in providing us with the following information:

  • Request for Consultation form
  • Office Notes (related to Orthopaedic Surgery diagnosis)
  • Diagnostic Reports (X-Ray, MRI, and other Radiology reports)
  • All films should be hand carried by patient to their clinic appointment
  • A referral or authorization for the visit from the primary care physician should be faxed with the Request for Consultation form

As soon as we receive this information, our staff physicians will review the medical documentation to determine the appropriate clinic and appointment date for your patient. On average, this process may take between 7-10 working days. If we do not receive completed information, it may delay the referral process.

Once the appointment has been scheduled, we will contact the patient by telephone or mail. You will also receive an outpatient appointment notification via fax for your office records.
