So, you're interested in submitting a program for continuing
education credits? We're happy to take you through the process.
You will need to fill out the following forms when
you begin to plan your CE activity:
Prior
to program planning:
Contact an ESN liaison. If an ESN liaison
is not involved in the planning, no program forms will be
considered. Call 734.763.5283 to identify your liaison.
Program
planning:
- The very first form to fill out is the Basic
Information form.This online form
asks for program name, date, planner name, planner
email, and ESN liaison.
- The online Checklist is required
to track program forms. Select the title of your program
by clicking here,
then choose the Checklist button.
The following forms are Microsoft Word documents (instructions):
A grey box will appear asking if you'd like to:
- Open the file or
- Save to your computer
If you select "open", you may either simply view it or fill
the form out.
If you choose to fill it out:
- After completion, go to the menu bar and select file
> save as and
save to your own filespace.
- Go to your email program, and send the form as an attachment to:
If you select "save to your computer", save it to your own filespace.
You can either fill it out immediately or go back at another time.
Again, once you've completed it, save and send as an attachment to:

- Proposal (6 pages)
- Educational Activity Documentation (1 page)
Choose one version:
- Biographical Data (1 page)
- CE Faculty and Planner Disclosure (1
page)
- Evaluation:
After
program completion:
- Select the title of your program by clicking here,
then choose the Statistical Data Sheet button.
Remember that you must provide evidence of listing in MLearning.
- Submit a copy of the CE certificate distributed.
- Submit a copy of the participant record, if all participants
are not entered into MLearning.
- Submit a copy of the evaluation summary, if available.
