Internal Requests (UM employees and affiliates)

  1. Complete the form below.
  2. We will send you a reply email stating that the request has been received. If the requested videotape is not available, we will let you know of its status.
  3. We will send the requested videotape, along with a letter, sign-in sheet, and accompanying handout (as applicable) through campus mail. Or, you can pick up this material at our office.
  4. Videotapes are available for a two week check out period.
  5. Return the videotape and completed participant list through campus mail or drop it off at our office.

External Requests (non-UM employees and affiliates)

  1. Complete the form below.
  2. We will send you a reply email stating that the request has been received. If the requested videotape is not available, we will let you know of its status.
  3. Please mail us a check in the amount of $15 per video (+6% tax where applicable), payable to the University of Michigan. Send the check to:

    Ann Arbor, MI 48105-5792
  4. Upon receipt of the check, we will mail the requested videotape, along with a letter, sign-in sheet and accompanying handout (as applicable). Or, you can pick up the material at our office.
