The University of Michigan Health System sponsors a Children's Memorial Service each year for children who have died while a patient here or at home after leaving our care.
Invitations are mailed out and this web form is intended as a convenient way for families to respond. If you have received an invitation and would like to respond, please RSVP by filling out the information below by April 14.
If you have not been invited to this service but would like to receive an invitation please send an email to chaplain@med.umich.edu. For proper planning of the service these requests must be received no later than April 1st. Requests received after April 1st will be included in next year's mailing. It would be helpful if you included "Please invite me to the Memorial Service" in the subject line.
A Remembrance Video will be shown during the memorial service. If you would like one or two photographs of your loved one to be included in the video, please complete the following information (PDF)
