The University of Michigan Health System sponsors a Children's Memorial Service each year for children who have died while a patient here or at home after leaving our care.
If you would like to attend, please complete the below RSVP form.
If you are a UMHS employee and would like to participate in the service please email umhs-chaplain@umich.edu with your name and title.
A Remembrance Video will be shown during the memorial service. If you would like one photograph of your loved one included in the video, please complete the photo submission form (PDF). In order to have a photograph of your loved one included in the video at this year’s event, you must RSVP and we must receive your photograph by April 9, 2013.
