You can schedule an appointment in two ways:
- By phone – call 734-615-3267
- By fax – fax the referral to 734-615-4674
Please be sure to have/include the following information:
- Child's name, date of birth and diagnosis
- Contact numbers for family including home, work and cell phones
- Concise past medical records faxed to our office within 24 hours (if making a phone referral)
