  • Teams and/or individuals can be nominated
  • Anyone can make a nomination

In an effort to formally recognize the ongoing patient and family centered care practices and initiatives that are happening everyday at Mott, a PFCC Recognition Award Program has been created. The program's premise is based on the "HOPE" bunny logo and inspirational story - Helping Our Patients' Everyday.

The HOPE award is an annual recognition award that honors those who demonstrate an outstanding commitment to PFCC. Individual staff and family volunteers, as well as teams, are eligible for nomination. Nominations are due June 30, 2009.

Selection Process

The PFCC Awareness & Visibility committee will narrow the entries to a group of finalists; the PFCC, PFAC & the PFCC Executive committee will chose the winning individual and team entries. An awards ceremony will be held for the finalists at which time the winners will be announced.

The HOPE Award

The individual winner of the HOPE award will receive a HOPE logo embossed leather portfolio and $1500 to use towards the annual IFCC conference registration and fees. The team winner will receive a celebration lunch served by the PFCC Executive Committee and a HOPE trophy to display in their area for the entire year. The nomination period for the annual award will be in June, with the award ceremony in September. Winners will be encouraged to present to internal leadership groups, as well as external venues.
