Application & Instructions

Only complete applications will be processed.  A complete application includes:

• The UMMS application with the home school official's signature of approval
• Official medical school transcript;
• UMMS Immunization record completed by a health professional; please do not attach personal immunization records.  A TB test must be administered within one year of your anticipated elective(s).
• Registration fee of $100 (refundable if elective is full or canceled).  Make checks payable to the University of Michigan.  In-state students (MSU & WSU) are exempt from this fee. 

Instructions

• Complete the online web application and click "Submit Application"
  NOTE - You must include a headshot image file with maximum size of 50K in order to submit your application
• Print the completed application, have it approved and signed by your school official
• Mail complete application to

 Incomplete applications will not be reviewed so be sure to submit all required documents together. Please allow at least 8-10 weeks for processing.
