Simulation Results for the Michigan Model of Diabetes Progression 11-May-2009
To validate the Michigan Model of Diabetes Progression, we applied the model to a simulated sample of 1138 subjects corresponding to the conventionally treated cohort of the United Kingdom Prospective Diabetes Study (UKPDS). The results presented here reflect statistics for 100 repetitions of a 10 year simulation for the simulated sample of 1138 individuals. The statistics includes mean, standard deviation (SD), median, min and max values with respect to repetitions of the simulation. Values for a single repetition correspond to the calculation method defined in the row heading that are explained below.
The table below shows the statistics for the 100 repetitions of the 10 year simulation considering natural progression of the disease. The statistics shown in the first columns are the mean, std, min, median, and max results for the entire simulation time period considering the 100 repetitions. The last set of columns shows means over 100 repetitions while focusing on specific time periods. Note that the column statistics per repetition are calculated after the row statistics (per individual / time) have been calculated for a certain repetition of simulation.
Note that the model for retinopathy is still under development; the results for blindness do not match between the simulated results and the UKPDS observed results.

Legend:
- Start Step and End Step: These specify the start and end steps or cycles that are being summarized. When the start step is equal to the end step, the results apply to the single cycle shown: zero refers to the initial state of the sample before any cycle is started. When the end step is greater than the start step, the results are summarized over the cycles included in the interval. For example, 6-10 is a summary of the cycles from 6 through 10 for subjects who enter cycle 6 and 1-10 is a summary of all the subjects from cycle 1 through cycle 10. Usually a cycle corresponds to one year of elapsed time.
- Rec. Count (Record Count): The total number of subject-years at risk in the interval defined by the start and stop steps. When there is only one step, it is the number of subjects entering the cycle. When there is an interval, then it is a sum over the number of subjects alive at the beginning of each cycle. Note that a person dying in a certain simulation cycle is counted in that step, yet not counted in the record count for the next step.
- Dem. Count (Demographic Count): Counts the number of subjects alive at the cycle listed in Start Step. Note that cycle zero is a dummy to describe the initial sample and no subjects can die during this cycle.
- Sum All: The sum of the values for the variable listed in the row title over all the records in the simulation interval defined by Start Step <= Step <=End Step. In general this is appropriate for Boolean variables; i.e., those that are zeros and ones. For Booleans such as state indicators it is the number of people-cycles having this condition in the simulation interval; when the interval is only one cycle, this number divided by demographic count is the prevalence of the condition in this cycle.
- Avg. All: The average of the values for the variable listed in the row title over all the records in the simulation interval defined by Start Step <= Step <= End Step, i.e. equivalent to Sum All / Rec. Count. In general this is appropriate for continuous covariates such as BMI. However, it should primarily be used when the interval contains only one cycle.
- Avg. Dem.: The average of the values for the variable listed in the row title over only the records of the alive individuals entering the simulation interval starting at Start Step, i.e. equivalent to Sum (Variable over all alive individuals) / Dem. Count. This is the same as Avg. All when the interval contains only one cycle equal to Start Step.
- Sum LOCF (Sum of Last Observation Carried Forward): The sum of the variables in the last record for each individual before or at the End Step. This sum is over all subjects who entered the first cycle. Note that all subjects who died before or at the End Step are included as long as they were alive at the start of the simulation. This is useful to count the number of people that had the condition at any time during the time intervals in which they participated even if they died.
- Avg. LOCF (Average of Last Observation Carried Forward): LOCF is the value for the variable either at the last cycle in the interval (End Step) or at the time of death for those who died before the End Step. Avg LOCF is the average of the variables in the last record for each individual. This sum is over all subjects who entered the first cycle. Note that this includes all the individuals who died before or at the End Step as long as they were alive at the start of the simulation. This calculation is useful to reflect average of accumulated numbers such as the average of the total cost spent on an individual regardless of death.
- State names represent variables that are Booleans which indicate whether the person is in that state. Row titles with State names ending with Entered represent Boolean variables that are one when the person was an incident case and entered that state in a specific year. Counting entered states is similar to counting incident cases whereas counting regular states is equivalent to counting years spent in a specific state.
