Custom Requisition

Materiel Services will work with you to customize an individual order form, commonly known as a Custom Requisition. We will preprint the forms to include your most commonly requested items and all other necessary information. You are only required to indicate the items requested. We request you submit a fax to MSC at 6-4029, by 3:00pm Monday- Friday if the Custom Requisition items are to be delivered the next day. Friday orders will be delivered on Monday. Orders dropped off after 3:00pm are not guaranteed next day delivery. Questions about Custom Requisitions may be directed to any Customer Service Representative at 6-6077 24 hours a day 7 days a week.
