A 80-year-old, otherwise healthy widow, is seen because of the recent onset of mild exertional dyspnea.
Physical Examination: BP 150/70, Pulse 110 and grossly irregular, R 16. No signs of congestive failure. No cardiac enlargement. The following was heard on auscultation.
