A 70-year-old maintenance worker with a large inferior myocardial infarct is about to be discharged from the hospital on the fourth day when he suddenly develops severe dyspnea, weakness, pallor and diaphoresis. Physical Examination: BP 90/60, P 80+reg., R 30. Bilateral crackles posteriorly. The following was heard on auscultation (it was not present on morning rounds three hours earlier).
