A 50-year-old office worker comes to the emergency room with severe dyspnea and mild diffuse anterior chest pain of one hour's duration. No previous symptoms. PMH. Negative. FH. Negative. SH. Noncontributory. An EKG shows only sinus tachycardia. No evidence of MI. Physical Examination: BP 90/60, P 110, R 20. Mild jugular venous distention. Bilateral moist crackles. No cardiac enlargement.
