Referrals

Managed Care Insurances (i.e. HMO, PPO, etc.) require a referral and/or authorization from your Primary Care Physician to the Specialty Care Division that your appointment is with. It is mandatory that your University of Michigan Registration Number (located on your blue UM registration card) is on the referral/authorization. If you do not have a UM blue card, please make sure your date of birth is on the referral. Your Primary Care Physician can fax this directly to our Document Control Center at 734-615-1139 or toll-free at 1-877-294-0123. Patient appointments will not be scheduled until the referral is received by the Document Control Center.

Medicaid Participants:
The University of Michigan Medical Center is located in Ann Arbor, Michigan, within Washtenaw County. Before the appointment is scheduled, Medicaid out of country or out of state will need to obtain a written referral and authorization to be seen by a Specialty Physician.
