A description of the main goals of the initiative:

Statewide quality collaborative designed to address adverse events in hospitalized medicine patients. 

A brief discussion of the scope of the project, and the interventions and metrics, if appropriate:

The initial focus is on VTE prophylaxis, but additional issues will be addressed in the future.  The VTE initiative is focused on risk assessment and appropriate prophylaxis for at-risk patients.  No specific interventions are dictated by the collaborative, but extensive data collection is required by all participating hospitals.  Data reports are provided to the hospitals and they are required to make use of the data in implementing quality improvement strategies.  The next topic will likely be PICC use and appropriateness.
