CMS Core Measures

A description of the main goals of the initiative:

This project aims to improve the performance of hospitalist on CMS core measures for CHF and PNA.  This will be accomplished by coordinating with CAs as well as improving hospitalists’ awareness of these CMS core measures.

A brief discussion of the scope of the project, and the interventions and metrics, if appropriate:
Metrics:

CMS Core measures:
CHF: Pts with EF < 40% must be on ACEi or ARB, if not, reason must be documented; Pts with CHF receive appropriate discharge instructions
PNA: Appropriate Abx use for pts with PNA; appropriate culture results for pts with PNA

Interventions:

Educational campaign to increase awareness of CMS core measures, coordination with Office of Performance Assessment and Clinical Effectiveness (PACE) and CIDSS, coordination with CAs to monitor and improve performance
