Hospitalist Led Antibiotic Stewardship

A description of the main goals of the initiative:

This project aims to improve the documentation and utilization of antibiotics on the hospitalist service. This will be accomplished by improving hospitalists’ awareness of their antibiotic use, including considering and documenting the indication, duration and day-of-therapy or start date for all their patients on antibiotics.  Additionally with the support of clinical pharmacists, an antibiotic time-out to address these issues as well as the ability to potentially tailor or discontinue antibiotic therapy will be initiated.  Lastly, strides towards improving the accessibility of guidelines to help direct therapy will be commenced.

A brief discussion of the scope of the project, and the interventions and metrics, if appropriate:

For patients on antibiotics, the % of progress notes, discharge summaries and service sign-offs that include indication, duration, and day-of-therapy or start date; the % of patients who had an antibiotic time out.
For MFH physicians, the % of MDs who perform the antibiotic timeout with the pharmacists


Educational campaign to increase awareness of these issues, coordination with Pharmacists to perform antibiotic time-outs, increased accessibility of guidelines, incentivizing these activities
