Diabetes Disease Management Program

GOALS

  1. To improve care and decrease length of stay for inpatients with diabetes mellitus.
  2. To identify patients with complicated or difficult to control diabetes who may benefit from a limited period of more intensive outpatient management, including education and case management.
  3. To provide increased clinical availability for patients requiring acute diabetic care, including education and case management.
  4. To provide diabetes education in both the inpatient and outpatient settings.

Inpatient Component:

The Disease Management Program will identify eligible patients admitted with the following diagnoses/clinical situations:

• Diabetic ketoacidosis or hyperosmolar coma
• Consultations initiated by the vascular surgery service
• Previously identified high utilization of health care resources

These patients will receive:

• Inpatient endocrine consultation for diabetes management
• Diabetes Education
• Outpatient endocrine follow-up (if needed) within 3 business days

Outpatient Component:

Patients seen as part of the Inpatient Component of this proposal will be eligible for short-term outpatient endocrinology follow-up prior to reestablishing care with their current provider (or referral to a new provider.)
