In order to receive the special rate of $130 plus tax per night (single or double occupancy), reservations must be made before the room block is filled or before the cutoff date of Wednesday, August 20, 2014, whichever comes first. Target Audience
This conference is intended for physicians, nurse practitioners, physician assistants, nurses, clinic staff, physical therapists, and social workers.

Registration Fees
Registration is encouraged by Wednesday, September 10, 2014 to plan for sufficient refreshments/lunch for all attendees and required to purchase a printed version of the syllabus (electronic syllabus is included in the course registration fee). Payment must accompany registration. Checks should be made payable to the University of Michigan in U.S. currency. Payments by American Express, Discover, MasterCard or Visa are also accepted. Fees include continental breakfast, coffee service, lunch and electronic syllabus for participants. On-site registration will also be available.

You will be emailed the link to the electronic syllabus at least three days before the course. Cancellation Policy
Refund requests must be received in writing, no later than September 10, 2014. An administrative fee of $50 will be deducted from your registration payment. No refunds will be made thereafter.

Accreditation & Credit Designation
The University of Michigan Medical School is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The University of Michigan Medical School designates this live activity for a maximum of 5.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Applications have been submitted to the American Osteopathic Association and the American Academy of Family Physicians for credits. An application has also been submitted for CEs with the Michigan Social Work Continuing Education Collaborative. Determination of credit is pending. Other credits by specialty may apply.
