Transportation
Motor vehicles are prohibited on the island and all travel is by carriage, bicycle, horseback or foot. Mackinaw City and St. Ignace are easily accessible by car. Transportation is available from either location to Mackinac Island by ferry or charter flight to the Island’s airstrip. Discount ferry coupons for transportation to Mackinac Island will be emailed several weeks prior to the course. Pellston Regional Airport serves Mackinaw City and is located 16 miles south off I-75. You must pre-arrange your ground transportation to Mackinaw City.

Target Audience
The conference will be of particular interest to physicians providing adult primary care and to cardiologists. Other health care professionals, including nurse practitioners and physician assistants who work with cardiology patients, will also find the information helpful.

Registration
You are encouraged to register as soon as you are certain of attending, as we cannot guarantee educational materials for applicants who are not preregistered and prepaid by Friday, August 5, 2016. Payment must be received at time of registration. The course fee includes educational materials, continental breakfast, coffee service, and private reception for participants, faculty, and families. Please RSVP for the reception on the Course Registration Form.

The private, evening reception at Grand Hotel on Friday, August 19, 2016 is included in your registration fee. Please make sure to RSVP when your register. Dress Attire: In accordance with Grand Hotel tradition, evening wear is required in all areas of the hotel after 6:30 pm: Dress, skirt and blouse or pantsuit for ladies, and coat, necktie and dress pants for gentlemen.

Cancellation Policy
An administrative fee of $50 will be deducted from your registration payment. Refund requests must be received in writing, no later than Thursday, August 11, 2016. No refunds will be made thereafter.

Accreditation & Credit Designation
The University of Michigan Medical School is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The University of Michigan Medical School designates this live activity for a maximum of 12.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Applications have been submitted to the American Osteopathic Association and the American Academy of Family Physicians for credits. Determination of credit is pending. Other credits by specialty may apply.
