Michigan Visiting Nurses works in conjunction with physicians and Hospital Discharge Planners to develop a plan of care that will provide a smooth transition from the hospital and continue the expert care found at the University of Michigan .

Our care coordination includes scheduling nursing and therapy visits, providing business support to include insurance eligibility questions, emergency planning, and care coordination with physicians' offices.
