We provide care to the chronically ill for patients who are under the care of multiple specialists and are having trouble complying with new medications and treatments, can have access to a primary care provider who can “put it all together” in the comfort of their own home. We can provide a second pair of eyes for the primary care physician. Often a primary care provider in the home can effectively evaluate the health status and capabilities of the patient and caregivers. We can improve access to care. Michigan House Calls will see patients for routine follow up visits to assess compliance to treatment plans, medication regimens, and to catch health issues before they become emergencies.
