End of Life Care
Home Care Services offers a wide range of care and services for those patients and families facing end of life issues. Michigan Visiting Nurses, Michigan Visiting Care, and the House Calls Program will work with the patient, family and the patient’s primary care physician to develop a care plan that will promote disease management, comfort care, and patient advocacy.
Michigan Visiting Nurses offers skilled intermittent nursing and rehab care that will assist patients and families in facing problems associated with end of life and improving quality of life. Services include:
- Assessment and monitoring of symptoms
- Patient and family education
- Evaluation of the home environment
- Coordination of care among physicians and care providers
- Nursing care ranging from general medical care to specialized infusion and wound care
- Mental Health Nursing
- Physical, Occupational and Speech Therapy
- Medical Social Work
Medicare and other insurance plans cover most services provided by Michigan Visiting Nurses.
Michigan Visiting Care offers a variety of services that are also in harmony with the palliative care philosophy. Michigan Visiting Care employs Aides who have been trained and are talented at assisting patients and families with personal care, light housekeeping, companionship, light meal preparation, respite care and companionship. Our goal is to help individuals maintain their independence and quality of life in the comforts of their own home. Initial assessments are completed by a highly trained and educated nurse supervisor and are free of charge.
Services range from 4 hour shifts up to 24 hour shifts, and on an individual basis. Most services are not covered by insurance, however Michigan Visiting Care works with patients and families to explore financial support based on need.
Our House Calls Program is designed to provide advanced medial and palliative care in the comfort of the patient’s home. Our Nurse Practitioners will provide an assessment, medication management, diagnostic testing and laboratory services and assist in care management with the patient’s other medical team and family members. Services provided by the House Calls Program are covered by all insurance plans.
If at any time the patient wishes to be referred to hospice services, our staff will provide them with information and refer them to the hospice of their choice.
