How to Make a Referral
An inpatient referral can be made by contacting your Discharge Planner. To make a referral from a hospital other than University of Michigan Hospital, you may call our Intake Department at 734-975-7441 or 800-862-2731 (extension 7441). Information that will be needed in order to process the referral include:
- Patient Name
- Address of care and phone numbers
- Date of birth
- Insurance information
- Type and start of therapy
- Name, phone number of the following physician for home care orders.
- Visiting nurse agency name and phone number
If the referral is sent after 5:00 p.m. we will process the referral the next business day. If the referral is sent on Saturday, Sunday or a holiday, please call the main phone number of the agency you wish to refer to. An on-call representative will contact you within one hour of your call to speak with you about the referral submission.
