What is Home Care and Transition Services

The University of Michigan Health System Home Care Services are a collaborative team of specialized departments with the common goal of serving the patients of the University of Michigan Health System (UMHS) in their homes or during the transition to home care from the hospital. These departments will work in tandem or alone to provide quality care in the home.

The University of Michigan Home Care services team consists of several departments – HomeMed – infusion provider and licensed pharmacy, MedEQUIP - medical supply company, Michigan House Calls – providing primary care in the home, Michigan Visiting Nurses – home nursing service. All services can be accessed via this web site (e-mail), phone or fax.
