Geriatrics Outreach Clinics

Geriatrics Center Outreach Clinics offer primary health care services in the local community. The primary care is provided by a Nurse Practitioner who has a Masters degree in nursing and special training in providing health care to older adults. The nurse works with and under the supervision of a physician. Please call The Geriatrics Center Clinics at 734-764-6831 for more information.

Appointments

Appointments may be made by telephone through the Geriatrics Center Clinics, 734-764-6831, Monday through Friday, 8:00a.m. to 5:00p.m. or during clinic hours at each site.

Clinic Visits

Please plan to be at the clinic 15 minutes early. Bring all of your medication, medical insurance cards, and your University of Michigan Health System 's registration card, if you have one. If you do not have a U of M Health System's registration number, the receptionist will register you and the card will be available at the next visit.

If you have questions or problems when the outreach clinic is closed, you may call the Nurse Practitioner through the Geriatrics Center Clinics at 734-764-6831.

Fees and Billing

Charges for office visits and complete exams are determined by the time spent and vary with the severity or complexity of the problem. Do not hesitate to discuss fees or your bill with us.

Services

  • Physical examination including pelvic examination and pap smear.
  • Management of chronic health problems and adjustment of treatment programs.
  • Collection of blood and urine samples and electrocardiograms.
  • Immunizations such as flu and pneumonia vaccines.
  • Treatment of common illnesses.
  • Assistance in adjusting your health care program to your particular way of life.
  • Counseling for good nutritional health and for chronic problems such as diabetes, high blood pressure and weight control.
  • Instruction in basic hygiene practices to prevent common infections, health problems and care of common problems for aging skin.
  • Cancer screening including prostate examination and instruction on breast self-examination.
  • Instruction on exercise and relaxation methods.
  • Counseling in personal and/or family problems.
  • Referrals to appropriate resources at the University of Michigan Health System and within the community when necessary.
  • Activities designed to improve and maintain physical and emotional health.
