Inpatient and Affiliated Programs The Geriatrics Center's inpatient activities include a multidisciplinary Geriatric Inpatient Consult Team based in University Hospital. Staffed by professionals from the Division of Geriatric Medicine, the team includes faculty physicians, geriatric fellows, and a clinical nurse specialist. The Geriatric Inpatient Consult Team works with the hospital staff to optimize care for the older adult. An Elder Life program for older hospitalized patients in the University of Michigan Health System is designed to help them maintain their physical, mental and emotional well-being. The program utilizes volunteers to provide stimulating activities, early mobilization, nutrition support, and relaxation interventions for frail elderly patients at risk for developing delirium while hospitalized. The program uses modest, but creative approaches to provide support to the nursing care that enables older patients to remain optimally functional during a hospital stay. Important outpatient clinical geriatrics programs are also carried out at the Veterans Affairs Medical Center in Ann Arbor , Michigan , which is home to the Geriatric Research, Education and Clinical Center (GRECC). The GRECC strives to provide new approaches to health care delivery for the geriatric veteran population. For older adults needing extended care, the Geriatrics Center and University Hospital participate in a preferred provider relationship with Glacier Hills, a life-care retirement community and nursing center in Ann Arbor, Michigan. GRECC also has an Extended Care Center. In addition, Arbor Hospice, headquartered in Ann Arbor, Michigan, provides home and hospice care to seriously ill patients of all ages.
A community based out-patient clinic, New Hope Outreach Clinic, offers health care services dedicated to meeting the medical needs and respecting the cultural sensitivities of African American older adults and their families. The clinic's primary mission is to reach underserved African Americans, but is open to all older adults.
