Affiliated Programs Glacier Hills Nursing Center The Geriatrics Center and University of Michigan Health System participate in a preferred provider relationship with Glacier Hills Nursing Center. A nonprofit, community-based nursing facility located in Ann Arbor, Michigan, Glacier Hills offers independent and assisted living units as well as intermediate and long term care in a 160 bed nursing facility. The University of Michigan patients at Glacier Hills are followed by faculty, fellows, and a full-time nurse practitioner from the Division of Geriatric Medicine. Glacier Hills has allocated 80 skilled beds to admit geriatric patients for sub-acute care focusing on rehabilitation. Collaboration with faculty and staff from many U-M Health System departments is an important component of this activity. This program has had an impact by reducing hospital placement days as well as providing an effective post-discharge rehabilitation program for elderly patients. The average length of stay for patients whose primary need is rehabilitation is approximately 20-40 days.
