New Hope Outreach Clinic
New Hope Outreach Clinic was created to offer health care services dedicated to meeting the medical needs of diverse adults. It is designed to work in conjunction with the many services available through the University of Michigan Health System and Michigan Home Care Services.
New Hope Outreach Clinic is a collaborative effort staffed by the University of Michigan Health System employees from Home Care Services and the U-M Geriatrics Center. Our heath care team includes a physician, nurse practitioner, social worker, and medical assistant specialist. These professionals work together to evaluate and treat health problems that become more common with aging.
The primary care is provided by a nurse practitioner who has a master's degree in nursing and special training in providing health care to older adults. The nurse practitioner works under the supervision of a geriatric physician. A geriatric social worker and a medical assistant specialist are also members of the health care team. The team provides primary care, care coordination, medication management, symptom treatment, mental health evaluations, health screening and education, social work, counseling, and referrals.
Clinic staff may assist older adults with transportation to the clinic, provide help with Medicare/Medicaid and other financial concerns and help with prescription assistance programs. The social worker is available for both clinic and in-home counseling to patients and their families.
Whether your condition is routine or chronic, one call and our professionals will work with you, your family, your primary care physician, as well as our specialists or other health care professionals to decide on the plan of action that is best for you.
If the need arises, we will see you in the comfort of your own home. We will also help to arrange other services including psychiatric nursing, physical therapy, home nursing care or care from home health aides.
The clinic is open on Mondays 8:30 A.M. to 12:00 P.M. and Fridays and 1:00 P.M. to 5:00 P.M.
