New employee health assessments

EHS provides health assessments for employees hired into the University of Michigan Hospitals and Health Centers or who transfer to another department. These health assessments are a mandatory part of the hiring process and should be completed prior to the first day of "Michigan Tradition and Values" training. At the visit, your immunization status and other relevant health information will be assessed, and you will receive a tuberculosis skin test; in addition, your vision may be tested (color vision screening), and you will be assessed to see if you need to be fitted with a mask to use when working with respiratory isolation patients. The health assessment is individualized to the specific occupational needs of your job.

After you accept the job offer
Once you have accepted a job offer, your hiring supervisor will complete and submit a New Employee Routing Form to Employee Health Service. This form contains employee health information about your new position and job duties.

The hiring supervisor then directs the new employee to the registration desk. All local calls should be directed to 734-936-4990, and all long distance calls should go to the toll-free number at 866-452-9896.    

Once the hiring supervisor has submitted your new employee routing form and you have completed your registration, please schedule your appointment for your health assessment.

For an appointment
Call EHS at 734-764-8021 during our regular business hours. We will make every attempt to schedule an appointment at a time that is convenient for you. The visit will take approximately 30 minutes, depending upon the requirements for the position.

Bring records
Please bring records of childhood and other immunizations (vaccines) that you have received. Usually, such records can be obtained through your primary care physician. If you have worked in health care, you may be able to obtain these records from your previous employer. Bringing the records with you will save the time and effort of having your blood drawn. Employees of the Hospitals and Health Centers must have proof of certain immunizations and TB screening. This is essential to maintaining a safe environment and complying with state and federal regulations and accrediting organizations.
