Your patients with a biopsy-confirmed diagnosis of skin cancer do not require a referral to U-M Dermatology. Appointments can be scheduled by you or your patient at any time during, before or after treatment. However, appointments for patients with possible skin cancers (NOT biopsy-confirmed) do require a consult request be provided to our General Dermatology Service.

Biopsy-Confirmed Skin Cancers / Mohs Surgery

For biopsy-confirmed melanoma, appointments can be made by calling the University of Michigan Multidisciplinary Melanoma Clinic (M.D.MC) at (734) 936-6360.

For biopsy-confirmed non-melanoma skin cancer, appointments can be made by calling the Cutaneous Surgery and Oncology Mohs Unit at (734) 936-4068.

General Dermatology

Please read our Consult Request Guidelines carefully before referring your patient to UM Dermatology. An evaluation by the referring physician is required prior to referral.

Instructions for UMHS Physicians:
Please complete the Outpatient Consult Request Form and fax it to (734) 647-6593. Our staff will contact your patient to schedule an appointment. For urgent appointment requests, please call the clinic (734-936-4054) so that we may accomodate your request immediately.

Instructions for Physicians External to UMHS:
For appointments at the the Taubman Center or Reichert Health Building, please call our appointment call center at (734) 936-4054 and at the prompt press 1. For appointments at Dexter Family Medicine, call (734) 426-2796. Be prepared to provide the following information:

• Patient's name
• Birthdate
• Contact phone number
• Address
• Insurance information
• History of treatment
• If available, patient's blue UMHS card registration (CPI) number

If urgent, please call our office, (734) 936-4054, M,T,W,F - 8:30am-5pm, Thursday 10am-5pm. We will call your patient to set up an appointment upon receiving this information.

Cosmetic Dermatology & Laser Center
