The American College of Cardiology Acute Myocardial Infarction Guidelines Applied in Practice (GAP) and Related Projects

From the completion of a very successful pilot project in early 2001,the American College of Cardiology (ACC) Acute Myocardial Infarction (AMI) Guidelines Applied in Practice (GAP) initiative in Michigan has expanded to include the Flint - Saginaw Project with five hospitals and the SE Expansion project with 18 hospitals. There are now a total of 33 hospitals involved with the ACC AMI GAP project with the potential to impact more than an estimated 40-50% of the patients with AMI in Michigan. Each subsequent AMI GAP project has built upon the successes and lessons learned from previous projects. The quality improvement model has evolved into one that is based on the Institute for Healthcare Improvement's Breakthrough Series collaborative model offering more support to the hospital teams. Perhaps most noteworthy though, is the continued enthusiasm and the expanded leadership that the local cardiologists have demonstrated for the projects led by their professional organization, ACC, and Kim A. Eagle, MD, FACC.

ACC GAP/AHA Get With the Guidelines Project (GWTG)

A leadership team from both the ACC and AHA has proposed a project to test combining the strategies and tools from both of these national initiatives. Recruitment for hospital participation will be targeted in western and central Michigan in spring, 2003.

ACC GAP Initiative in Heart Failure

Led by Anthony DeFranco, M.D., and Todd Koelling, M.D., plans are being developed for an inpatient to outpatient GAP program to improve quality in patients hospitalized with heart failure. The Flint/Saginaw hospital collaborative and greater Flint Health coalition are the partnering institutions.
