Patient and Family Centered Care Program

At the heart of the U-M Cardiovascular Center lies the belief that providing the ideal care experience stems from a partnership between patients, their families, physicians and staff. As a result, the CVC has created a Patient and Family Centered Care Program, to provide a platform for patients and families (Advisors) to share their personal experiences and provide input on how the CVC can improve care for future patients and family members.
Core Concepts
Dignity and Respect:
- Listening to and honoring each family’s cultural and religious values, practices and beliefs throughout all phases of care
- Recognizing family strengths and different ways of coping
Information Sharing:
- Communicating and sharing complete and unbiased information
- Providing timely and accurate information to assist in decision making
- Setting the proper expectations
Collaboration:
- Providing opportunities for patients, families and staff to work together to create and implement optimal policies, programs and services
- Inviting patients and families to participate in the education of staff, faculty, and students through sharing of personal experiences

Participation:
- Encouraging patients and families to participate in decision-making at their level of choice
