Proposal process
An individual -- or groups of individuals representing one or more departments -- submits a proposal via the on-line application. Once submitted, each proposal is then sent for review to members of the Community Health Services Review Committee. Each member evaluates and scores the proposal. After the proposal is sent out for review, the proposal developer(s) may be asked to come to the next Community Health Services Task Force meeting to give a five- to ten-minute presentation on their proposal and to spend an additional five to ten minutes answering questions from the reviewers and other committee members. A notice is then sent to the proposal developer(s) regarding the committee's final decision.
When there are more requests than funding available or a proposal request exceeds $2,000, the following actions may be taken:

Partial funding may be awarded.
- Proposal developer(s) may be asked to reduce the scope or costs of the project.
- The committee prioritizes requests according to: determination of need, previous awards to the same developers, the number of community members served by the project, the benefits for the community and UMHS, and other criteria deemed necessary.
- Funding is denied. (Note: Proposals may be re-submitted at a later date.)
Deadlines
Funding requests are due on the first day of the odd months (January, March, May, July, September and November). If your proposal is approved, you will be notified by the 15th of the month. Projects must be completed within one year of the funding award -- or funding will be allocated to other projects. Projects that have not been fully funded on the first request will be considered for subsequent funding according to the following guidelines:
- 2nd requests for funding will be awarded for up to 75 percent of the maximum amount
- 3rd requests will be awarded up to 50 percent of the maximum amount
When an invoice/account number is needed for payment, the Committee Chair, Sharon Redmer, must be notified in order for payment to be authorized. Ms. Redmer will determine if the recommendations of the Committee have been met, and if the scope of the project and costs have remained as defined in the project proposal.
Project evaluation
In order to measure the effectiveness of the project, the proposal developer must send an evaluation summary within one month after the project/event is completed. This evaluation should show:
- Population served
- Number of participants
- Brief summary of services provided
- Evaluation results
- Followup
Previously funded Community Health Services projects
The guidelines for submitting previously funded events are the same as stated above. Requests must be made using the on-line submission form. These requests are reviewed in the same manner as previously submitted proposals.
