Instructions & Forms
Biomedical Research Store
|
A completed Order Form is required to purchase products from the Biomedical Research Store. Limited storage space prevents us from holding stock products awaiting pickup, so Stock Product Order Forms must be submitted IN PERSON at the location of your choice. All locations are open 8:15am-12:15pm and 1:15pm-5:00pm, Monday through Friday.
Over 50,000 items are not stocked but are included on our contracts. To place a special order, complete a Special Order Form and submit in person or VIA FAX to your desired location. Please review the special order policies as outlined on the form and verify all information before submitting. Special orders generally arrive within one day, and must be picked up at the location where the form was submitted.
All Order Forms require each item's catalog number included on our Stock Products webpage, or the catalog number for special orders can be obtained from vendor websites listed under Useful Links.
All purchases and their accompanying Order Forms require a shortcode, the only
method of payment accepted by the Biomedical Research Store. (A shortcode is a
six-digit number that summarizes the chartfield information for your account as
designated by the University.) Please have your shortcode with you when you wish
to make a purchase. If you are unsure of which shortcode to use, please contact
your appropriate department personnel. Store personnel are not authorized to
look up shortcodes. You will receive a receipt at the time of your purchase, and
your shortcode will be billed semi-monthly by Service Unit Billing and appear on
your Statement of Activity. If you have questions regarding current or past
charges to your account, please call 763-4715.
