University of Michigan Adult Bariatric Surgery Program

All patients must meet the criteria outlined below to be considered a candidate for the UMHS Bariatric Surgery Program. Please carefully review the criteria before registering.

  1. Must be between 18 and 60 years of age. Requests for patients between 61-65 years will require approval by bariatric surgery director and medical director. For patients under 18 years of age, visit the Pediatric Comprehensive Weight Management Center website.
  2. Body mass index of 40, or greater, or 35 with life threatening co-morbidities (hypertension, heart disease, diabetes, pulmonary hypertension, sleep apnea, hyperlipidemia, polyarthritis).

  3. Documented compliance with a non-surgical weight loss program for a minimum of 6 months with visits documenting weight, diet, exercise, and lifestyle modifications within in the past 2 years. (Note: This may vary based on individual health insurance carrier's requirements.)
  4. Medical evaluation and documented work-up to rule-out underlying treatable causes for morbid obesity within the past year.
  5. Psychological evaluation must be done by a qualified mental health provider, knowledgeable in bariatric surgery, within 6 to 12 months of the appointment with the surgeon.
  6. Must attend at least one UMHS bariatric surgery program gastric bypass information session. This must be attended prior to enrolling in the program.
  7. Weight loss surgery is not to be performed in the presence of certain medical and psychological diagnoses. If you have one of the following conditions below, you are ineligible for bariatric surgery.
    1. Pregnancy, lactation
    2. Active substance abuse
    3. End-stage cardiovascular disease
    4. Severe or uncontrolled psychiatric disorders
    5. Anorexia

    The following may also make you ineligible for surgery:

    1. Unstable medical condition
    2. Kidney disease
    3. Active binge eating disorder or bulimia nervosa
