Referral Guidelines

  1. You must be over 18 years old. For those under 18 years of age, please visit the Pediatric Comprehensive Weight Management Center website.
  2. You must have a Body Mass Index (BMI) of 40 or greater (or 35+ with weight-related health diseases such as hypertension, heart disease, diabetes, polyarthritis, pulmonary hypertension, sleep apnea, and hyperlipidemia). Calculate your BMI.
  3. You must have documented compliance with a medically supervised weight loss program with your primary care physician for the length of time specified by your insurance carrier. Typically, the documentation period is for a minimum of six months in length within the past 2 years. The documentation must include all diet, exercise, weight loss medications and lifestyle modifications you have attempted. NOTE: Criteria may vary based on your health insurance carrier's requirements.
  4. A comprehensive Health Maintenance Examination and documented work-up to rule out any underlying treatable causes for morbid obesity within the past 12 months.
  5. You must attend at least one UMHS bariatric surgery program informational session. You must attend and return your completed medical information packet to us prior to scheduling our program specific evaluations.
  6. Absolute contraindications to having weight loss surgery include pregnancy, lactation, active substance abuse, end-stage cardiovascular disease, severe or uncontrolled psychiatric disorders, and anorexia nervosa.
  7. Relative Contraindications which may exclude you from having weight loss surgery include unstable medical condition, end-stage renal disease, active binge eating disorder, or bulimia nervosa.
