Outpatient Referrals and Consultations





A referral can be made by Michigan Medicine Physicians through MiChart.
Simply search for: “Bariatric Surgery”

To qualify for weight loss surgery:

Age > 18 years
(No max age limit)

BMI of 40 or more
or
BMI of 35 or more
and
one or two weight-related illnesses such as:
Diabetes
High blood pressure
High Cholesterol
Sleep apnea
Asthma
Heart disease
Arthritis
Acid reflux



Please Note:
Due to higher complication rates, BMI must be less than 65 to be considered for surgery.





Insurance Requirements



Most insurance carriers have additional requirements prior to bariatric surgery. Requirements may vary from patient to patient and are also subject to change. We encourage that all patients contact their insurance carrier with regard to their own coverage. We have provided a list of important insurance questions that patients can use to help guide their conversation with their provider




Medically Supervised Weight Loss Documentation



Some insurance carriers require 3, 6, or 12 consecutive months of documentation. This may be accomplished by a primary care physician or referring physician. We have provided a medical weight loss form below for your convenience. If your patient is within the Michigan Medicine health system, they may attend our dedicated clinic for medically supervised weight loss prior to surgery by calling 734-936-5738.




Primary Care Physician Letter



Insurance carriers will request a letter from the patient’s Primary Care Physician (PCP) or referring physician in support of your patient’s choice of undergoing bariatric surgery within our comprehensive program. For your convenience, we have provided a letter that you can use.
